Patient Registration Process

We welcome requests for registration from patients living in or moving to the practice area.

Patients can register by completing a HSCR 1 form. This form will ensure that we register only those patients who are entitled to treatment on the NHS.

This form must be completed in full with any part that’s are not applicable to you clearly marked as N/A

Along with this form you will need to provide photographic id and proof of address, examples of what is acceptable are listed on the form but if you have NONE of these, please come back and talk to us and we will try to find an alternative.

Once this form has been completed and returned we may arrange a New Patient Registration appointment with a GP to ensure we have the necessary clinical details to hand as it can take 6-8 weeks for patient notes to arrive from the previous practice.

This form is available in several languages.  Please follow the link.

All newly registered patients should attend for a health check within 28 days of registering to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

This assessment will include weight, height, urine testing and blood pressure reading.  Details of past medical history and family history will be taken along with dates of vaccinations & smears etc.  When attending this appointment please bring a sample of urine.

Medical treatment is available from the date of registration. Please contact reception for further information.

Please be aware the practice has a policy for patients who regularly do not attend.  This can be found on the appointments page.
